We would like to invite you to participate in a second round of the Minnesota COVID-19 Antibody Study (MCAS), an important study on the impact of the COVID-19 pandemic that NORC at the University of Chicago is conducting on behalf of the University of Minnesota. You may have previously completed research activities for MCAS or have been nominated to participate by someone in your household. We would like to know about your continued experiences during the COVID-19 outbreak.

The Minnesota COVID-19 Antibody Study (MCAS) is a study of the residents in Minnesota and their exposure to COVID-19, the disease caused by the SARS-CoV-2 virus. The study is being conducted by NORC at the University of Chicago in partnership with University of Minnesota. The study is funded by the Minnesota Department of Health.

YOU are eligible for MCAS because you or someone in your household agreed to be contacted again after completing a survey about COVID-19 with NORC at the University of Chicago. Your participation is important because, by testing your blood for antibodies, we will learn more about the percentage of people in Minnesota who have antibodies against SARS-CoV-2.

We have completed our first round of data collection where we asked respondents to complete the antibody test kit. We are now in our second round of data collection and we are asking you to complete a short, 10 minute survey about your experience in the pandemic and we will send you a $10 gift card code as a thank you for your participation. At the end of the survey we will ask you to participate in the antibody test kit portion of the survey and collect a few drops of blood. We will send you an additional $25 gift card code as a thank you for your time.

We want to know more about your experience during the COVID-19 pandemic, including questions about the COVID-19 vaccine. We will ask you to tell us more about the people in your household. We may ask to include other adults in your household in this study if they are eligible.

If you have children who are school-age, we may also ask some questions about their experience and invite them to participate in the antibody blood test. We will offer each child $25 for their participation.

Absolutely! Whether you are new to the study or you didn't have the time to participate in round one, your participation in invaluable in round two. We hope you will participate!

The goal of the MCAS is to better understand the spread of COVID-19 in Minnesota by measuring samples of blood, from folks all around Minnesota, for antibodies to SARS-CoV-2, the virus that causes COVID-19. By testing for antibodies we hope to learn more about the presence and spread of COVID-19 in Minnesota.

It will take under 10 minutes to complete the survey and 20 minutes to collect and return your blood sample to us.

NORC at the University of Chicago maintains the highest standards of confidentiality.

No information obtained in the course of this study may be disclosed in a manner in which the individual supplying the information is identifiable, except to a very small number of authorized staff on MCAS and at NORC, and then only for study administrative purposes, not for dissemination. Any violation of your confidentiality is subject to penalties of up to five years in prison and/or as much as a $250,000 fine.

All of your information is treated as confidential.

Participation in MCAS is also entirely voluntary, and there are no penalties for not participating. A respondent's survey and test results are never reported individually, but instead are grouped with responses and results from other persons in the study to create statistical and analytical reports. In short, at no point does NORC or MCAS identify individual respondents and never publishes individual responses or results.

Yes, you may enter that address and we will mail your COVID-19 Antibody test kit to you at that location.

You can still participate in the survey and receive the $10 gift card for your participation. At the end of the survey we will ask you about the antibody test kit and you can decide whether you will participate then. We hope you will participate fully, but you don't have to collect a blood kit in order to participate in the survey. We would like to leave the door open in case you reconsider.

We will give you step-by-step instructions with pictures to help you. The finger prick is much like the blood test that many people with diabetes do in their own home. It is designed to be as simple as possible.

The antibody test has been designed to be as simple as possible, with little to no discomfort for you. There will be a little discomfort upon pricking your finger that normally resolves quickly.

If it is helpful to have someone look at the instructions and help you, sure.

Yes, you will receive the results to your COVID-19 antibody test. After we receive your sample and process it, we will mail you your results to the physical address on file. If you have any questions about your results, please contact MCAS toll-free at 1-877-393-9734 or send an email to mncovid19@norc.org.

Thank you for offering, however, it is important to the research that the measure is taken the exact same way by all respondents. This includes using the same equipment.

You will get one $10 gift card code for completing the survey and we normally send that about one to two weeks after we receive your survey. If you complete an antibody test kit, we will send you a $25 gift card after the lab confirms they have received it.
